In addition to the membership fee there is an initial $100 non-refundable enrollment fee.

When enrolled you commit to 3 month continued membership in order to gain the full experience of everything we have to offer to you.

We ask for 30 days written notice for terminating membership.

At this time, membership is not available to beneficiaries of any government healthcare program 
(Medicare, Medicaid, TRICARE/CHAMPUS/CHAMPVA, the VA, Indian Health Service, etc.)

Members recognize that our membership does not replace health insurance and will not cover charges on care or services received outside of those listed in the Stone Oak Direct Primary Care membership.
